About Affinity Whole Health
Affinity Direct is the telehealth platform of Affinity Whole Health LLC — a Midwest clinical system with physical locations in Cleveland, Columbus, Indianapolis, and Chicago. With years of experience delivering in-person clinical care, Affinity Whole Health built its online platform to extend access beyond clinic locations.
The goal is straightforward: make it as easy for a patient in rural Ohio to access provider-evaluated, FDA-approved treatment as it is for someone walking into one of our clinics. That means real providers, real prescriptions, and real medications — not a questionnaire-and-checkout process with no clinical oversight.

As of January 2025, Affinity Direct is licensed and operational in:
